Diagnostic Work-Up

Table 63-1 lists the likely causes of illness (differential diagnosis). A clinical diagnosis of encephalitis was considered based on the evidence of brain lesions. Investigational approach may include

• Cultures of blood and CSF for bacteria and Cryptococcus neoformans

• Serology. A significantly high titer of IgG specific for Toxoplasma gondii in a single serum may support the diagnosis of Toxoplasma encephalitis.

• In failed investigation, the following tests can be undertaken: • Histology of brain biopsy

• Detection of genetic material by PCR in CSF and biopsy
